Pricing and Insurance

Planning for medical care can be stressful.
​We want to eliminate any surprises.

Visit Pricing

​All payments for services are due at the time of your appointment. We accept cash, check, debit and credit. Insurance policies are listed below.
Appointment Pricing
Fees are based on total time spent on a patient's visit. This will always be more than the time spent face-to-face because it includes time spent before a visit for tasks like reviewing intake forms and/or previous records/labs as well as time spent after the visit on things like ordering labs, sending in prescriptions or supplement recommendations, and completing chart notes.
Example: for a New Patient Visit that lasts 75 min, patients can expect the total time spent to be at least 95 min, with more complicated cases taking longer.
Price ranges for new patient visits (FOCs) are listed below with the corresponding procedure codes for insurance billing included: 
90-104 min FOC (99205 + 99417x2)= $375

105-119 min FOC (99205 + 99417x3)= $400
120-134 min FOC (99205 + 99417x4)= $425
135-149 min FOC (99205 + 99417x5)= $450
150-164 min FOC (99205 + 99417x6)= $475

Example: for a Follow Up Visit that lasts 45 min, patients can expect the total time spent to be at least 65 min, with more complicated cases taking longer.
Price ranges for follow-up visits (ROCs) are listed below with the corresponding procedure codes for insurance billing included:
10-19 min ROC (99212) = $95
20-29 min ROC (99213) = $125
30-39 min ROC (99214) = $165
40-54 min ROC (99215) = $225
55-69 min ROC (99215 + 99417) = $300
70-84 min ROC (99215 + 99417x2) = $325

Other Charges
Returned check fee: $25
No show/cancellation fee: $75-$150
- This will be charged if a patient fails to arrive within 15 min of the scheduled appointment time or cancels with less than 48 business hours notice. For example, an appointment scheduled for Monday at 2:00PM must be cancelled by Thursday at 2:00PM the week prior. Late cancellation/no-show fees are as follows:
  • $75 for appointments scheduled for 15-29 min
  • $100 for appointments scheduled for 30-59 min
  • $150 for appointments scheduled for 60-90 min​ ​​

Insurance Policies

For insurance companies that we are in-network with, we will bill your insurance for you.

For insurance companies that we are NOT in-network with, we will not bill your insurance directly.  If requested, we will provide you with the necessary paperwork needed to submit to your insurance company for possible reimbursement for your appointment.  We are not able to give you specific information about what reimbursement you can expect, as coverage benefits vary from plan to plan.

If you would like to contact your insurance company for more information on what reimbursement you can expect, it may be helpful to share the above procedure codes which are most commonly used for visits. (Please note that exact procedure codes cannot be determined until after your visit, therefore these codes are subject to change.) We recommend asking your insurance company if they cover the additional time code (99417) and if coverage varies if a visit is telehealth vs. in-person, so that you avoid any surprise bills.
